    Why would they want a name and e-mail address for a survey? Would it not prove to be a little more tactful if they had a question stating:

    May we contact you about your responses?

    - Yes
    - No

    Then if yes, fill in that info? I think they are more likely to get a greater number of responses of the survey was anonymous by default. That reminds me...

    An excerpt from the survey's intro at the top: "...If you would like to have us provide you with additional information about our new high speed wireless internet service, please include a contact name and email address."

    But it is not optional. You can't submit the survey unless your name and e-mail is in the text box. That's what I'm saying.

    oh ok, I didn't know that ... :-[ Then I agree, it shouldn't be a required field(s), I think I'll pass along the suggestion.

    I agree Xeno. I was filling out the survey until i reached the end and saw that they wanted my name and email address. So I didn't submit it.

    We have spent a lot of time testing different types of customer premises equipment and sources of bandwidth (VSAT, CWJ, Merit, etc.). We will be offering the service to the public in about 2 weeks time. The prices have not been finalised but we are looking at an installation cost of US$150 and a monthly cost of US$50 for a 256/128 package, this is to be confirmed.

    Re: survey form that requests a name and email address, we wanted to be able to provided people with feedback when the service is ready. We must apologise for not making it optional, we have been under pressure, sorry.
