.pdf --> Csv?
On the US Embassy's site when signing up for a visa u fill out a pdf and submit. I assume that info is recorded in a DB or CSV.
I want to create something similar.. where u fill out a form and the info is recorded into an excel file.. (csv format, doesnt matter)
Did a little research and found FDF (Acrobat Forms Data Format)
Read that you can use Adobe GoLive, Acrobat Pro, and some others. Think I'll use GoLive to create the form, but has anybody ever done this? Little help needed

What i would like know is, is there any signicant benefit in using a PDF form instead of a normal HTML form.

The format. HTML page would render the form differently.
I want the same layout as the paper document. So if u should print the form it displays like a regular paper appl.
